情報プライバシーに配慮した地域医療連携における患者情報の共有のありかた

本研究の目的は、地域医療連携における情報共有について、情報プライバシー、その中の自己情報コントロール権の観点から共有する情報の範囲と共有の相手を患者がどのように決めていけばよいか、そのために医療従事者は患者にどのような説明をすればよいかを明らかにすることである。
令和3年度は、地域医療連携が展開されている地域中核病院から診療所にどのような情報が提供、共有されているのかを把握することを目標とし、地域中核病院の地域医療連携室などを担当する医療従事者を対象としたインタビュー調査のための倫理審査手続き、ならびに調査依頼を行った。
4施設から20名の医療従事者、主に看護師と社会福祉士の協力を得ることができた。調査は、インタビューガイドに沿って、地域中核病院から診療所へ提供される情報項目と情報の詳細さ、地域医療連携で生じている情報プライバシー上の懸念や問題、患者への説明内容と同意取得方法などを聴取した。分析は、インタビュー内容から逐語録を作成し、質的記述的に分析を行う。分析途中ではあるが、地域医療連携の場における情報連携について、個人情報に対する医療従事者の意識は二極化していることがうかがえ、とても敏感に反応する人が一部いる一方で、そんなことは全く意に介さない人もいることが示された。調査は、令和3年11月から開始し、現在も継続している。
また、先行研究の成果の一部を第41回医療情報学連合大会（愛知県・名古屋市）においてポスター発表を行い、本研究を進める上での有用な意見を得ることができた。 -
認知症者のセルフマネジメント力を支えるケアプログラムの作成と有効性の検討

認知症初期・軽度認知症診断後の当事者本人が感じている気持ちや必要としている支援の把握については、その方との関係性も重要なことから時間をかけつつ認知症カフェや当事者の会等を通じ継続的に収集を行った。当事者の方からの語りでは、認知症という病気はテレビからの情報などからのイメージでとらえていることが多く、まさか自分がという気持ちと、これからの自分がどうなってしまうのか、何をすればいいのか混乱したという内容が多かった。しかし、当事者自身に病識がない場合と何らかの異変を感じて診断を受けた場合、また家族構成や、担っている役割や価値観によって生じる悩みは異なり個別的であった。今後さらに事例数を増やし少しずつ関わりを通して聞き取りを行い、分析する必要がある。
認知症診断時に関わりケアを行う看護師に対し実際の現場でどのような情報提供や支援を行っているかについて調査するための前段階として、認知症看護認定看護師に聞き取りを行った。その内容の概略として、認知症疾患センターでは診断が主となるため、接する時間が少ない中で当事者のその人らしさや気持ちの把握が難しいため、表情や言動に注意し予測しながらの関わりとなっている。また、物忘れ外来などの診療場面でも介護者からの介護に対する悩みや大変さなどの話が中心となり傾聴が大切と思いつつも時間的制限もあり、介護者だけでなく、ゆっくりと当事者に関わる時間を持ちたいがそれも難しいという葛藤がある。かかりつけ医やケアマネジャー、ケースワーカーとの連携の中での看護の役割についても、各機関や地域の特性やシステムによりさまざまであることがうかがえた。以上の内容を加味しつつ調査を進める。 -
急性期における認知症患者の尊厳への配慮の実態と患者の思いとの一致度について

本研究の目的は、急性期看護を実践する看護師が認知症患者に看護援助を提供する際の尊厳への配慮の実態と、患者の尊厳に対する思いの一致度を看護師の代理回答から確認することにより、急性期看護における認知症看護の課題を明確にすることである。
令和元年度は、急性期病棟で、認知症患者に対する看護援助を提供する際の尊厳への配慮の実態を明確化、および「急性期病院認知症看護尊厳評価表」の項目抽出のためのインタビュー調査を実施した。調査のため、所属施設と調査実施施設の倫理申請をし、承認された。調査対象は、関東にある200床以上の急性期病院に勤務する臨床経験年数3年以上の看護師5名である。調査は、インタビューガイドに沿って、看護援助を提供する際に尊厳に配慮する点、急性期病棟であるため尊厳への配慮が困難な点、患者の尊厳への期待・満足度の判断の際に参考にする情報などについて聴取した。あわせて、認知症看護ケアチェックリストへの回答と、日本語版患者尊厳尺度に患者の思いを想定して回答を依頼した。調査期間は令和2年2月から実施し、現在も調査を継続している。分析手順は、インタビュー内容から逐語録を作成し、質的記述的に分析を行う。結果をもとに、ドラフト版急性期認知症看護尊厳評価表を作成する。作成する評価表は、研究分担者とともに内容妥当性の評価を行い、認知症認定看護師および老人看護専門看護師とともに表面妥当性の評価を行う予定である。 -
Development of the community-based, integrated nursing care system applied with the dementia total care program

A multiple regression analysis was used data collected from 308 nurses involved in care for older people. The analysis showed that professional autonomy in nursing was related to the Sense of Coherence Measurement Scale (β = .313, p < .001), Scale of Nursing Practices for Elderly with Cognitive Impairment with the Aim of providing Person-centered Care in Acute Hospitals (β = .276, p < .001), and length of years working as a nurse (β = .138, p < .01) (R2 = .23).
This study provided 90 nursing staff with educational interventions related to decision-making support for people with dementia and management of physical complications for people with dementia. The results show the necessity to correctly assess the decision-making ability of older people under the care of multidisciplinary cooperation in a hospital. The findings suggest that certified nurses in dementia nursing need to cooperate with ward nursing teams to continuously assess the decision-making ability of older people. -
中山間地域において１６年間継続してきた高齢者水中運動講座の効果の縦断的検証

開催して21年目に入る高齢者水中運動講座の実施と、これに係る研究を学内の様々な分野の教員が相互に連携しながら実施した。本年度も研究の土台となる体制の構築を更に進めた。この為の１つとして、研究者らが運営・研究を実施するための必要な知見を、「学会への参加」「研究から得られたデータの有効活用するために地域行政機関と連携をはかる」などして各研究者が積極的に推進した。しかし、新型コロナ感染防止の為、2020年3月から水中運動講座自体を開催できない状況となった。この為、毎年3月に実施している体力測定会が開催できなかった。
年間を通した水中運動教室の運営における参加者の観察で、本年度も数か月の間に認知機能の低下が疑われるようなケースも見受けられた。このようなケースの把握のために、運営にあたるスタッフ間での情報共有するシステムが必要で、昨年度はこの試みの模索を進めたが、システムの構築までには至らなかった。そこで本年度は、このシステムの構築をはかるとともに、地域行政と連携した体力測定会の開催の準備を2月まで進めた。前述のように体力測定が直前になり開催できなかったため、この連携の有効性については検証できなかったものの、検討したプロセスについては従前よりも大きな進展が測れた。
参加者の第一目的が自身の運動機能的な健康のプロモーションのためであることなどから、今後もより有用・有効な仕組みや環境の構築・改良していく必要がある。このほか、体力測定会で蓄積されたデータについて、認知機能や転倒予防などのいくつかに焦点を絞った解析を現在すすめている。 -

Examination of the weighting coefficient of the dementia nursing care checklist 1st edition score calculation, the individual attributes of the target person related to the correction factor (clinical experience, expertise, suport, Satisfaction with one's dementia care practice, gender, position etc) As a result, when we looked at the total score of dementia nursing care, it was found that it is desirable to adopt it as a correction factor in attributes such as clinical experience, expertise, cooperation, and satisfaction/confidence of dementia care.

This study aimed to develop a tool to assess dementia patients' dignity based on proxy answers from his/her family members and nurse. We conducted a questionnaire survey with 146 pairs of a patient aged over 65 years, his/her family member, and a nurse in charge. Exploratory factor analysis confirmed 11 items in 3 categories on expectations regarding patients' dignity. A multiple regression analysis revealed that 11 items had significant correlations between answers of patients and those of their family member and nurses, and a simple regression analysis revealed 11 significant correlations between patients' and their family member's expectations of dignity, although only one significant correlation between patients' and nurses' expectations. As to satisfaction of dignity, a multiple regression analysis revealed 9 significant correlations among these pairs, while a simple linear regression analysis revealed 13 correlations between patients', and their family member's responses.

In this study we implemented and evaluated the program to improve the quality of care for situations encompassing features from dementia prevention to end of life care. It was established that maintaining and improving physical functions, particularly the ability to walk is important.
The program was effective in sustaining the mental functioning of participants involved for 5 years or longer. The frequencies of consultation as requested by family caregivers were mostly for patients at moderate stages of dementia, suggesting the need to establish and create a consultation framework for patients who are at the early stages of dementia. The quality of care provided for patients at the facilities varied depending on the career and specialties of care managers employed, suggesting the necessity to involve nurses with long career. As the individual elderly patients have specific hopes and intentions for the terminal lives, it is important to provide care by respecting such intentions. -

1）The water exercise classes have been held 103 days totally during 2012 and 2014, including 31 times of 1-class-day and 72 times of 3-class-day, and the number of total attendants was about 5000 persons. 2）All data were analyzed about the activities of 14 years, and several articles were published. 3）At the end of every year measurement meeting was held to the attendants of class and other elderly people living around Komagane City, and in this meeting, bone density measurement, questionary survey using health scale, examination of cognitive function, and the measurement of knee extension muscular strength were made. Analyzing these prospective data, the effect of water exercise was determined. 4）As the tool of nursing education in the field of gerontological nursing and qualified class of dementia nursing, attending the water exercise class was shown to be very effective for the students.

This study aims to understand the situations around the dementia care provided by nurses for elderly patients with dementia in hospitals, and also examine factors influencing the execution of this care to alleviate ethical dilemmas among nurses. We created a dementia care check list, where nurses rate care items in terms of ‘importance’ and ‘frequency of use’. The analysis yielded 29 items grouped into five factors for the ‘importance’ of dementia care and 28 items grouped into six factors for the ‘frequency of use’. The reliability and construct validity of the items was confirmed. For the relationship between ‘frequency of use’ and demographic characteristics, nurses who reported that they had specialized knowledge as well as nurses who reported that they had no help from superiors and colleagues showed higher frequencies of use.

A Study on End-of-life Care Strategies for Old People with Dementia Living in Group Homes

We conducted a questionnaire survey at group homes for old people with dementia throughout Japan, and investigated the present situations and problems involved in end-of-life care by comparing the group homes that employ nurses with those that do not and the backgrounds of the facilities. An interview was also conducted regarding collaboration with medical institutions, and particularly efforts to secure the quality of medical care for end of life. The findings showed that half the facilities had experience providing end-of-life care and viewed collaboration with medical institutions as a difficult problem. The facilities provided medical care by collaborating with nurses, both in-house and outside nurses. They strived to improve their staff's skills through various workshops provided both in and outside of the facilities due to the necessity to conduct sufficient assessment and provide appropriate information for medical institutions in order to facilitate collaboration with medical institutions.

The objective of this study is to describe details and methods of end-of-life care in accordance with advance directive for old persons with dementia in nursing homes. We surveyed nurse administrators of nursing homes throughout Japan through questionnaire, and found that efforts to obtain advance directive from elderly persons with dementia in nursing homes are very rare. Moreover, interviews with 25 nurse administrators about their successful experiences assisting with end-of-life care showed a clear understanding on how they can"collect information necessary for care planning", "apply the advance directive as requested by the elderly person", and "reflect on how they could have done a better end-of-life care".

A study for developing a nursing care skill model of end of life for elderly with dementia about nurses at the long term care welfare facilities

The purpose of this study was to clarify the model of toward improving nursing care skill for end of life of elderly with dementia about nurses at the long-term care institution.
The subjects were nurses (n=336) working at the long-term care institutions for the elderly with dementia in Nagano prefecture. Forty-seven and nine% of nurses answered the question. Their needs of nursing care skill for end of life of elderly with dementia were 15 parts. The courses of end of life care skill model were created eleven times in year. The aim of model creation was aiming at development of the care skill capability of nurses aiming at improvement in the quality of end of life care of the elderly people with dementia in institution. And it was carried out as the courses of end of life care, making up the guideline of end of life care. The courses of end of life care were carried out eleven times in year. The totals of participants were 18 nurses. The guideline (Q and A) of end of life care was printed as the booklet that was 50pages of 7 parts.
The model may be an effective way to development of the care skill capability of nurses aiming at improvement in the quality of end of life care of the elderly people with dementia in institution. Especially, the model had changed the perception and values of nurses who were rejected care of the elderly people with dementia in institution. Furthermore, this study has suggested that it was important to focus on care skill-up of nurses in the quality of end of life care of the elderly people with dementia. -

平成13年度に行なった入所条件要綱の調査のうち、入所評価基準で特徴的だった項目をもとに、質問紙(平成15年度に調査した入所基準および入所評価基準((1)入所希望者の心身の状況、(2)家族・介護者の介護力、(3)在宅生活の可能性、(4)住環境の状況)の作成を行なった。この調査の目的は、老人保健施設ならびに介護老人福祉施設において、どのような実態があるのか明らかにすることである。平成17年度は、質問紙の内容の検討の後、調査を行なった。
