集中治療室（Intensive Care Unit:以下ICU）入室患者の痛みは、主に急性痛である。急性痛は、患者のQOLを著しく下げるのみならず、身体的・心理的ストレスとなり、イライラや不安から興奮・不穏状態を呈し、全身状態の悪化にもつながる。したがって、クリティカルな状態の患者の看護において、痛みのコントロールは重要である。多くのICU入室患者は、人工気道や持続的な鎮静薬使用のため言語的コミュニケーションが困難であり、これらの尺度を使用しての痛みの把握は難しい。そこで、言語的コミュニケーションに頼らない痛みの評価尺度として、日本語版CPOT（CPOT-J）を作成した。CPOT-Jは、0～8点で評価し、得点が高いほど痛みが強いと判断する。作成した尺度を心臓血管外科患者に試用し、信頼性・妥当性・反応性の検証を行った。その結果、心臓血管外科患者の痛みを評価するのに有用な尺度であることが示唆された。しかし、心臓血管外科患者は手術創や留置されているライン類がほぼ同一であり、ほかの疾患の患者の場合は異なる部位の痛みがあると考えられる。そこで今回、心臓血管外科以外の患者にCPOT-Jを試用し有用性を検証する。

心臓手術後患者がICUで行う超早期リハビリテーションの安全性に関する研究

本研究の目的は、心臓手術後患者における初回体位変換直後から、循環動態パラメータが経時的にどのように変動するのかを明らかにし、看護師が体位変換実施の判断をするためのアセスメントを支える根拠を提示すること、および、実施後の異常の早期発見のための基礎データを得ることである。
2019年度からの、COVID-19の拡大・減少の繰り返しに伴い、ICUでのデータ収集は実施不可能であったため、国内外の文献レビューおよびデータ収集項目の精選を行ってきた。しかし、COVID-19の終息の見通しが立たず、大幅な研究計画の変更が必要であるため、研究課題に応じた研究デザインの修正・変更を検討した。現状において、病院およびICUでは面会制限があり、ICU内でのデータ収集目的での入室は難しいため、ICUでのデータ収集を行わない研究計画への変更を検討している。
また、昨年度は文献レビューより、「心臓手術後初回体位変換のアセスメントモデル」の作成を行った。今年度は、作成したアセスメントモデルの内容の見直しを行った。
「心臓手術後初回体位変換のアセスメントモデル：CAMP-AB4CD」は、CAMP＝基地…体位変換実施環境（C:基準・標準、A:患者の同意、M:マンパワー、P:医師の指示）、AB4CD…患者状態評価（A:出血、B:呼吸、C:循環、C:意識・不穏、C:カテーテル類、C:カテコラミン、D:苦痛）の項目を心臓手術後初回体位変換を安全に実施するための評価指標とし、体位変換実施前および実施後も経時的にアセスメントができる内容である。 -

The purpose of this study was to construct an assessment model to prevent pain, agitation, and delirium from patients in intensive care unit (ICU). We requested the directors of nursing service depart from 253 institutions which has certified medical specialists to participate in the study. 109 institutions have agreed to participate with a total of 2,531 nurses who received the survey and 1,133 have responded. The survey was about the years of experience, nursing certification, the pain, agitation, and delirium scale used, cognition of the pain, agitation, and delirium, and kind of nursing care provided. By this time, the significant difference among nursing certification and general nurses is revealed however, the construction of the assessment model is still on going.
