    高浸潤能を有する口腔癌は難治性であり、予後不良であるが、生検時に高浸潤能を把握し、そのメカニズムを解明することで、集学的な個別化医療を図ることができる。近年、原発固形腫瘍からの遺伝子診断に基づく薬剤感受性診断など、いわゆるオーダーメイド治療が進められている。一方で、「がん幹細胞」や「上皮間葉転換（EMT：Epithelial-mesenchymal transition）」の概念が浸透するに伴い、原発巣細胞の形質の不均一性（heterogeneity）の問題が浮上し、結果、細胞増殖に特化する胞巣内癌細胞と、運動性に特化する浸潤先端部側の癌細胞などが多様な「癌組織」を形成している。
    まず、TCGAデータベースの口腔癌病理画像から浸潤能が高い症例を選び、その遺伝子情報を網羅的に解析した結果、PTK7が高発現していた。その浸潤能に密接に関わるPTK7の機能解析を行うこととした。
    そこで、今回は口腔扁平上皮癌において浸潤能の様態を良く示すがん浸潤様式山本・小浜分類（YK分類）を用いて、PTK7発現がどのようにがん細胞浸潤に関わるかを調べ、そのメカニズムを解明することでがん浸潤の抑制化につなげられるか検討することとした。山梨大学医学部附属病院を受診した口腔扁平上皮癌患者より得られた病理組織切片80症例を対象として、PTK7を一次抗体とした免疫組織化学的染色法を用いて、その局在性と発現を臨床病理学的因子と比較し、検討した。その染色結果では、浸潤様式が高くになるにつれてPTK7発現が強く呈しており、浸潤・転移と正の相関を認めた。
    これらのことより、口腔扁平上皮癌におけるPTK7の強発現は、浸潤能が高くリンパ節転移を来しやすく予後不良であることを示唆し、予後予測因子として利用できる可能性が考えられた。

    Co-culture with NOF or CAF significantly increased the migration and invasion of both the low-invasive H357 and high-invasive SCC4 cells . The expression level of both Tks5 and MT1-MMP protein and the number of expression spots of Tks5 was significantly increased by culture with NOF-CM or CAF-CM compared with culture with SFM in both H357 and SCC4 cells. Moreover, the migrating and invasive abilities of OSCC cells decreased by inhibition of Tks5.
    These results were suggested that invadopodia formation is important mechanism for migration and invasion of OSCC cells and it is regulated by interaction between OSCC cells and fibroblast.

    Using immunohistochemical techniques to examine the expression levels of claudin-7 in 67 cases of OSCC, claudin-7 expression was detected in 35 (52.2%) of the 67 cases. Moreover, six cell lines with various invasive properties were investigated in vitro to compare mRNA and protein levels of claudin-7 using reverse transcription-polymerase chain reaction (RT-PCR) and the western blotting method. Decreased claudin-7 expression correlated significantly with T-category (p<0.05), lymph node metastasis (p<0.01), and mode of invasion (p<0.001). Patients with positive claudin-7 expression had a significantly better prognosis (p<0.05). Claudin-7 protein and mRNA levels were lower in the HOC313 and TSU cells, which have higher invasive potentials compared with other cell lines. These results suggest that loss of claudin-7 expression is associated closely with invasion and lymph metastasis and is an unfavorable prognostic factor in patients with OSCC.

    Maspin expression in oral squamous cell carcinoma(OSCC) was detected in 64.8%. Patients with positive maspin expression had a significantly better prognosis. In contrast, maspin expression was lower in mode of invasion(Yamamoto-Kohama Critria) 4D type with high invasive potential. Moreover, Maspin expression showed a negative correlation with uPA/uPAR expression in the uPA/uPAR system. Expression pattern, uPA(+)/uPAR(+)/Maspin(-), exhibited a stronger invasive potential and the worst survival rate than other invasive grades of OSCC.

    RATIONALE: Pyogenic spondylodiscitis is an infectious spinal disease that causes significant motor dysfunctions. Its diagnosis can be challenging owing to its rapid onset and nonspecific symptoms. PATIENT CONCERNS: A 79-year-old Japanese man with a history of type 2 diabetes mellitus and polymyalgia rheumatica presented to our department with tongue pain. Following partial glossectomy and wisdom tooth extraction under general anesthesia, on 10 postoperative day (POD) the patient developed right-sided abdominal pain and difficulty in walking. On 12 POD, the patient was admitted to a municipal hospital due to respiratory distress and paraplegia. DIAGNOSES: The patient was diagnosed with pyogenic spondylodiscitis and empyema. Blood tests revealed elevated C-reactive protein levels (36.5), white blood cell count (19,570), and neutrophil count (17,867). INTERVENTIONS: The patient received meropenem hydrate 3 g/2 days as empiric antibiotic treatment for acute infection. Upon admission to the emergency department on 16 POD, the lung abscess was drained, hemilaminectomy was performed. OUTCOMES: Blood cultures, sputum tests, and cultures from the thoracic and spinal abscesses drained during surgery revealed methicillin-sensitive Staphylococcus aureus. The infection was successfully managed, and the respiratory disturbance and inflammatory response improved. However, the lower half of the patient body remained paralyzed. Subsequently, the patient was transferred to a rehabilitation facility on 45 POD. The patient continued to undergo functional restoration training, gradually regained function, and eventually achieved the ability to walk with grasping gait. LESSONS: This is the first case report of S aureus causing pyogenic spondylodiscitis and empyema due to blood stream infection from a post-oral surgical wound. Pyogenic spondylodiscitis arising from a secondary hematogenous infection is difficult to diagnose and can lead to severe functional impairment. Prompt and appropriate diagnosis and treatment based on detailed patient interviews, additional blood tests, and computed tomography are essential.

    RATIONALE: Infections that spread to the pterygomandibular muscle can be misdiagnosed as temporomandibular disorder (TMD) because of the resulting difficulty in opening the mouth. Importantly, infection of the pterygomandibular space can extend to the skull base in the early stages, and a delay in therapeutic intervention can lead to severe complications. PATIENT CONCERNS: A 77-year-old Japanese man with trismus after pulpectomy was referred to our department. This case report describes a rare instance of meningitis with septic shock caused by an odontogenic infection, initially misdiagnosed as TMD due to similar symptoms, leading to life-threatening complications. DIAGNOSIS: The patient was diagnosed with sepsis and meningitis resulting from cellulitis in the pterygomandibular space caused by iatrogenic infection after pulpectomy of the right upper second molar. INTERVENTIONS: After emergency hospitalization, the patient developed septic shock and required blood purification. Subsequently, abscess drainage and extraction of the causative tooth were performed. However, the patient developed hydrocephalus secondary to meningitis and underwent ventriculoperitoneal shunting to alleviate the condition. OUTCOMES: The infection was controlled and the patient level of consciousness improved following treatment for hydrocephalus. The patient was transferred to a hospital for rehabilitation on the 106th day of hospitalization. LESSONS: Infections of the pterygomandibular space may be misdiagnosed as TMD, owing to the main symptoms of restricted mouth opening and pain on mouth opening. A prompt and appropriate diagnosis is crucial because these infections can lead to life-threatening complications. A detailed interview, along with additional blood tests and computed tomography (CT) scans, can aid in making an accurate diagnosis.

    As a discrimination method for estimating invasive potentiality and prognosis of oral squamous cell carcinoma (OSCC), the mode of invasion (Yamamoto Kohama-criteria) centered on the form of pathological tissue specimen has been known to be clinically useful, especially in Japan where it is used frequently. However, evaluation of this mode of invasion is based on subjective visual observation, which has created a large gap between evaluators and facilities using such the mode of invasion. This problem is such that unification of objective evaluations is a challenge. Therefore, this study aimed to develop a method of automatically determining the mode of invasion by medical image processing using digital images of the invasion front of OSCC. A shaped feature of the invasive fronts was extracted to create a classifier by Random Forest, a machine learning algorithm, in conjunction with the mode of invasion opted for based on the judgment of the clinician. As a result of inputting multiple test images to the created classifier, it was confirmed that the classifier outputs a decision that is very close to the judgment of the clinician. Therefore, automatic determination of the mode of invasion by a medical diagnostic imaging system was shown to be feasible and of high accuracy.

    Purpose: The purpose of this study was to compare bone regenerative capability following use of polytetrafluoroethylene (ePTFE) membrane against that when various densities of pore titanium meshes are used with and without low intensity pulsed ultrasound (LIPUS).
    Materials and methods: Adult male white rabbits were divided into 8 groups. In 4 groups, after incising along the nasal bone, four 3 x 8 mm bone defects were made in both sides and covered by an ePTFE membrane (group E: n = 15), a high density pore titanium mesh (group H: n = 15), a low density pore titanium mesh (group L: n = 15), and no mesh (control) (group C: n = 15). Furthermore, LIPUS was irradiated after surgery in 4 groups (groups EL, HL, LL and CL, in each n = 15). The rabbits were sacrificed at 1, 2 and 8 weeks postoperative, and formalin-fixed specimens were embedded in acrylic resin. The specimens were stained with hematoxylin and eosin. For immunohistochemical analysis, the specimens were treated with bone morphogenetic protein (BMP)-2 antibody.
    Results: Group H had significantly higher values than groups L, E, and C regarding bone area ratio and labeling index of BMP-2 positive cells (P &lt; 0.05). Furthermore, Group HL also had significantly higher values than the other groups regarding bone area ratio and labeling index of BMP-2 positive cells at 1, 2 and 8 weeks postoperative (P &lt; 0.05).
    Conclusion: The results suggested that high density pore titanium mesh could induce new bone regeneration more than low density pore titanium mesh and ePTFE membrane.     The purpose of this study was to evaluate the effects of Le Fort I osteotomy on nasal septum deviation and differences in left and right airway sizes, and to determine whether the nasal septum was affected by differences in the direction of movement. Forty patients underwent conventional Le Fort I osteotomy, and computed tomography (CT) was performed preoperatively, and 1 week and 1 year postoperatively. The nasal septum angle and airway area were measured at the anterior, middle, and posterior positions on the CT images Patients were divided into 2 groups depending on each difference in movement (impaction &gt;= 5 mm or other; anterior movement or other; and impaction asymmetry or other). There were no significant differences in the nasal septum angle and the airway of all patients. Among the patient groups, there were no significant differences in the nasal septum angle and airway. We concluded that conventional LI osteotomy did not influence the nasal septum deviation or the left and right airway asymmetry.     © 2016 Kunio Yoshizawa et al. A ranula is a mucus retention cyst or pseudocyst caused by leakage of mucus from the sublingual gland and generally occurs in the oral floor. In addition, drug induced hypersensitivity syndrome (DIHS) is a rare but well-recognized serious adverse effect characterized by fever, skin rashes, generalized lymphadenopathy, hepatitis, and hepatosplenomegaly and oral stomatitis. This paper presents the first case of successfully treated sublingual ranula with localized injection of OK-432 after healing from drug induced hypersensitivity syndrome, which has previously been unreported in the literature. We present the case of a 38-year-old Japanese woman with sublingual ranula that responded successfully to localized injection treatment with OK-432 after healing from drug induced hypersensitivity syndrome. She was affected with cutaneous myositis and interstitial lung disease when she was 26 years old. At the age 34 years, she received additional oral treatment of diaminodiphenyl-sulfone due to deterioration of the cutaneous myositis, which resulted in drug induced hypersensitivity syndrome (DIHS) with severe oral stomatitis. Local injection of OK-432 to the ranula may be a very safe and useful treatment method even if the patient has a history of drug allergy and has connective tissue disease such as cutaneous myositis.

    口腔扁平上皮癌の浸潤能および予後を推測するための判別方法として、病理組織標本の浸潤形態に着目したがん浸潤様式(YK分類)が臨床上有用であることが知られており、国内では頻用されている。その一方で浸潤様式の評価が、主観的な視覚所見によるものであり、評価者や施設間により大きな隔たりが生じ、客観的評価として統一しにくいことが問題である。そこで、本研究では浸潤先端部のデジタル画像からコンピュータ処理によって自動的に浸潤様式を判別する手法を開発することを目的とした。具体的には浸潤尖端部の形状特徴を抽出し、臨床医の判断によって付与された浸潤様式と共に機械学習アルゴリズムであるRandom Forestによって分類器を作成する。作成された分類器に複数のテスト画像を入力した結果、臨床医の判断と極めて近い判断を出力することが確認され、デジタル画像からの浸潤様式の自動判別が高い精度で実現可能であることを示した。(著者抄録)

    BACKGROUND: Vascular endothelial growth factor (VEGF) family members play a major role in angiogenesis and vascularization. VEGF-A promotes tumor angiogenesis by stimulating the growth of tumor vascular endothelial cells. In addition, VEGF-C has been identified as a potent inducer of lymphangiogenesis in tumor and lymph node metastasis. Previous studies have investigated the association between clinicopathological factors and the expression of VEGF-A and VEGF-C in oral squamous cell carcinoma cancer (OSCC), but the results are contradictory. In this study, we investigated the relationship between VEGF-A and VEGF-C expression and OSCC clinicopathological factors and prognosis.
    METHODS: Expression of VEGF-A and VEGF-C was evaluated in surgical specimens from 61 patients with OSCC and three human oral cancer cell lines (OSC-19, OSC-20 and HOC313) by immunohistochemical staining and enzyme-linked immunosorbent assay, respectively. We also determined the relationship between the 5-year survival rate and clinicopathological factors, such as TNM classification (Union for International Cancer Control, UICC), lymph node metastasis, recurrence, histological differentiation, location, and mode of invasion.
    RESULTS: VEGF-A expression correlated significantly with lymph node metastasis. VEGF-C expression was associated with lymph node metastasis, recurrence, and a poorer 5-year survival rate. A multivariate analysis demonstrated that VEGF-C is an independent prognostic factor for patients with OSCC. VEGF-C expression was significantly up-regulated in HOC313 cells compared to OSC-19 and OSC-20 cells.
    CONCLUSIONS: These results indicate that VEGF-C may be a predictive factor for OSCC outcome, lymph node metastasis, and recurrence. Moreover, VEGF-C may be an important factor in the development of new therapies for OSCC patients.

    BACKGROUND: Abnormalities in cell-cycle-controlling genes are important in the malignant transformation and proliferation of tumors. Among these genes, the tumor suppressor gene p53 is the most notable, and its mutations provide an indicator of tumor progression and prognosis. Proliferating cell nuclear antigen (PCNA) is a highly conserved nuclear protein that is expressed during cell replication and DNA repair. This study examined the expression of p53 and PCNA at the invasive front of oral squamous cell carcinomas (OSCC) by immunohistochemical staining, and investigated the relationship of these proteins to clinicopathological findings and prognosis.
    METHODS: Fifty-nine biopsy cases of OSCC were examined by immunohistochemical staining. Clinicopathological data were gathered and patient survival was analyzed.
    RESULTS: The p53 labeling index (p53-LI) and PCNA labeling index (PCNA-LI) were examined at the invasive front of the tumors. A high p53-LI (p53+) was observed in 17 of the 59 cases (28.8%) and a high PCNA-LI (PCNA+) was observed in 28 of the 59 cases (47.5%). Among the modes of cancer invasion, many of the p53+/PCNA+ cases could be confirmed as highly invasive cancer (P &lt; 0.05). In addition, the p53+/PCNA+ cases showed a high risk of tumor recurrence compared with the other expression forms, and patients with p53+/PCNA+ had a worse prognosis than those with the other expression forms. High labeling indices of p53 and PCNA are associated with poor prognosis in patients with OSCC.
    CONCLUSION: We suggest that it is important to investigate the expression of p53 and PCNA at the invasive front of OSCC. J Oral Pathol Med (2011) 40: 693-698

    The present study evaluated the relationship between alpha 3, alpha 6A, and beta 1 integrin expression in cancer cells at the invasive front of oral squamous cell carcinoma (OSCC) and survival rates, as well as the clinical and pathological characteristics. Sections of 100 specimens of primary OSCC were immunostained to assess alpha 3, alpha 6A, and beta 1 integrin expression in cancer cells at the invasive front. OSCC patients with higher expression levels of alpha 3, alpha 6A, and beta 1 integrin had significantly better prognosis than those with lower expression levels (median survival at low vs. high expression levels: alpha 3, 37.1 months vs. 55.7 months; alpha 6A, 38.3 months vs. 47.9 months; and beta 1, 26.1 months vs. 46.1 months) (P&lt;0.05). In addition, beta 1 integrin expression showed the highest correlation with clinical and pathological characteristics. This study concludes that alpha 3, alpha 6A, and beta 1 integrin expression in cancer cells at the invasive front are related to the mode of invasion and prognosis in OSCC.
